Extract the .img file and rename it to something dos-friendly such as MSFS214.IMG
Then from dosbox, do something like this:
dosbox -machine cga -c "mount c " -c "C:\RUN.BAT"
In RUN.BAT I do:
c:
BOOT MSFS214.IMG

Had this sim installed on my amstrad pc1640 in my childhood... Used to take off and let the plane fly straight line for ever till it ran out of fuel or trying to climb the highest altitude possible
