I had this, it came from a magazine coverdisk, probably Amiga Computing. Quite a fun Boulderdash clone, before the Amiga got an official conversion in the form of Rockford. I recall that it didn't load on the later AGA machines; there is a fixed version, I don't know if the download here is one.
