If anyone has trouble with running the game, I personally tried everything suggested below and the game wouldn't run for me until I set it on windowed mode.

This is one of the best, weirdest games ive ever found. Its stalker on LSD!! With vehicles! Its freaking long too! Right now im like at least 15 levels deep into the game. And its not shallow of content. Only one gun exists in the game which sucks but you eventually unlock a flamethrower attachment, a bolt gun attachment, flamethrower attachment, and probably more attachments as well, one attachment is called blower i havent tried it yet. And the gunshot sound is perfect. Surpisngly, this game is really NOT shallow on content in terms of enemy types and vehicles and level design. Its janky as all hell, but i was able to use hellforces d3d9 wrapper (you gotta rename inhabitedisland.exe to hell.exe or else it wont work). The game did not have 1920x1080 resolution but thankfully the hellforces d3d9 wrapper forced windowed mode. All games look HD in windowed mode!! So that made me happy. Theres one mission where you have to drive your jeep down a 3-4 mile long bridge where you see nothing but acid-water to the left and right of you and have to pray you dont get hit by mortar shells. I flipped over it was hilarious. Its hard. Play on easy. I have godmode enabled and there were still a bunch of parts i almost could not get past because a lot of the missions want you to protect stupid NPCs or something like that.
Notes. On the escort geologist mission. Make a real hard save every like 5 minutes or so. She runs into pebbles on the ground and dies instantly. I had a lot of fun running in front of her and shooting all the sticks/pebbles out of the way honestly.
Its just.. so insanely different than anything ive ever played before. Its irrational. Impossible. Bizzarre. Janky.. Yet somehow its like the best atmosphere i ever felt in my gaming existence.
i discovered some commands (from hellforces and trying stuff out)
noclip = 1
speed = 200 (90 is default run speed)
fog = 0 (i kinda want to do a whole new game after discovering this one! it looks soooo much better with fog disabled)
The game honestly does not crash. It just feels/seems like it would. Use vertex shader version 2 if version3 shows black textures everywhere!
MASTERPIECE OF WEIRDNESS!!! Oh the english translation files are included in a folder on the iso, u gotta manually place them/overwrite

Ooops, forgot, as Clyde3D sagely said, also uninstalled the up to dat PhysX I had, and installed instead the PhysX that comes with the game (version 9.10.0512)
The bad news is that the game doesn't have acceleration, for me at least, so the detail is rather poor.

Got it working (played various levels).
For me, the solution was this:
**Use config.exe, uncheck Music and check Unmute.
Don't activate musics in game options.
Also compatibility with Windows XP, Service Pack 3. And running it as Administrator.
That's it, the game finally began ok.

As a personal choice, I run it in windowed mode, plus BorderlessGaming.exe, the usual with some old games, so I can Alt-Tab without eventual crashes, and use Cheat Engine for bullets (4 Bytes, 35 default to 9999), infinite stamina and life (both Float, 0 to 100 max), freeze them checking the box for that.

Couldn't install in Win10 but not giving up yet.
Eventually will try with WinXP on Vmware for luck.
Wanted to send a huge HUG to Keith, aka wkduffy.
