How I got the game to work: Drag HTW.iso over DosBox program icon. After the C:\, type D: and hit enter. Type dir and enter to show directory of the iso file. Type config (which is the first exe file in the list) and enter to run game setup. Use arrow keys/enter to go through the setup, choosing 'Sound Blaster 16' as sound source. At the end, it will say 'Installation complete. To play the game, type HTW in the C:\HTW directory!' After hitting Ok, input htw and press enter to start the game.
I have zero understanding of Dosbox, so this was how I blundered through using online tutorials.
